Health Requirements
Massachusetts law requires immunization and vaccination documentation to be on file in the counseling and health services office for students attending Salem State University.
Before you start classes at Salem State University, you need to make sure the health insurance requirements are met and health information forms are on file with us. You can upload your immunization information on our health services portal, located in Navigator, or if you prefer, mail/fax to our office.
Residential students must submit their forms and be in full compliance with the immunization policy two weeks prior to moving into the residence halls. This policy applies to all new students entering in the summer, fall or spring. Full-time commuter students continue to need to be in compliance with the immunization policy prior to starting their semester.

Prevention Requirements
Salem State University requires every incoming student to complete three online prevention programs 鈥 one for alcohol, one for cannabis, and one regarding sexual violence prevention. These programs are confidential, online educational courses. Follow these easy steps to complete your mandatory training requirements:
Sexual Assault Prevention Workshop
The Title IX office at Salem State has launched an online, video-based interactive workshop on Healthy Connections, Relationships and Sexual Consent Awareness. This year, the online workshop will be available accessible through Canvas and will be available by mid-August, 2025. All students will be assigned to complete the training and will receive a notification via Canvas when it is available.
How to Access the Sexual Assault Prevention Workshop:
- Go To Canvas! The training will be available to all new and transfer students on your student Canvas account.
- Log into Canvas using your SSU student ID and password. On the left column, select 鈥淐ourses.鈥 From your Orientation course, select the "Modules" tab, then scroll down to "Online Prevention Program." There you will find, Sexual Assault Prevention Training for Undergraduates.
The workshop will be available until September 12, 2025 at 11:59 pm. All new and transfer students must complete the online workshop by this date. Once the workshop is available on your Canvas course page, you will receive weekly reminders to complete the course.

e-Checkup To Go: Alcohol and Cannabis
This is an online educational program about alcohol and cannabis. It is an assessment and personalized feedback program that includes Salem State-specific information. Please note: You must take each online program listed below 鈥 they are separate online programs. Each takes approximately 15-20 minutes to complete.

Follow the instructions below for both programs:
- Once on the e-Checkup website, select 鈥業 have NOT completed the e-Checkup To Go previously鈥, then select 鈥榖egin鈥.
- Complete all sections of the program and review feedback. Remember, this is confidential and only the verification of completion is recorded.
- After reviewing all information, select 鈥榁erify completion of your e-Checkup To Go鈥, input your information and select 鈥楴ew Student Requirement鈥 and submit.
